If this will be your first appointment with us, fill out the "First Appointment" form below.  If we've seen you before, use the "Current Clients" form.  We'll give you a call right away to set up your appointment!  We are open Monday through Friday from 8 am to 5 pm.  Payment is due at the time of your visit.
